How do I return school forms during the school holidays?
You can provide us with a copy of a recent school report. However, this will only be considered suitable for triage if there is adequate information to determine whether an assessment is appropriate. If this is not the case, our clinicians will need to await completion of the school forms when they reopen.
Is the procedure available via the NHS, self-pay or both?
The procedure is available through self-pay only. Minor Skin Surgery starts from £375 and will increase in price depending on the nature of your procedure. We will confirm the actual price of your procedure during the Initial Consultation, which costs £80.
Is the procedure a fixed cost?
The price of our Minor Skin Surgery appointments will vary depending on the person and factors, including the number of lesions to remove. There are no hidden costs, so you can decide if the procedure is right for you after your Initial Consultation with one of our clinicians.

A pair of custom ear moulds costs £50. An ear mould cost depends on whether they are made bespoke or are bought pre-made – the £50 fee at Provide Wellbeing covers the cost of creating an impression of your ears, choosing a colour, and then getting your custom ear moulds made while you wait.
What happens if my child is not diagnosed at the autism assessment?
After the ASD assessment, if the consultant cannot be sure of a diagnosis of autism, you may be referred to further specialists. Does my child need up to date medical records for an autism assessment?
Our autism assessment team will need consent from the parents or guardian to view the child’s medical records. The required consent form will be sent to you by a member of the Provide Wellbeing team and will need to be signed and returned to us in order to help review eligibility for the ASD assessment.
