NHS Right to Choose for Children and Young People (ages 6–18)

Autism, ADHD and combined assessments

Provide Wellbeing is now accepting NHS Right to Choose referrals for autism, ADHD and combined assessments. If you are registered with a GP in England, you have the legal right to choose where your NHS assessment takes place, meaning you can ask your GP to refer you to our specialist service for timely, high-quality support.

Our pathway also includes access to medication (where clinically appropriate) and psychoeducation, ensuring ongoing support beyond assessment.

Current Right to Choose status

At present:

  • We are not yet accepting NHS Right to Choose referrals
  • We will update this page as soon as referrals are able to open

Availability of Right to Choose services can also be affected by NHS activity and funding limits. You can find out more about IAPs (indicative activity plans) here: How NHS Activity Limits Work.

Planned Right to Choose pathways

Once NHS approval and mobilisation are complete, we expect to offer the Right to Choose pathways.

  • Right to Choose contract under the MSE framework

Each new pathway will be clearly explained on its own page once referrals are open.

To avoid delays or rejected referrals, we recommend waiting until the pathway is confirmed as open.

Is Right to Choose right for me?

Right to Choose can be an option if:

  • You live in England and are registered with an NHS GP.
  • Your local NHS waiting times are longer than you would like.
  • You have found an approved provider (like Provide Wellbeing) that has the capacity to see you.

What to expect from referral to assessment

As part of the NHS Right to Choose process, a set of clinical questionnaires and forms must be completed before an assessment can take place.

These are an essential part of the pathway and help our clinical teams to:

  • understand the child or young person’s needs and history
  • determine the most appropriate assessment pathway
  • ensure assessments meet NHS and NICE standards

How the process works

  1. A parent or carer can explore this pathway via our website or speak to their GP about a referral.
  2. The GP downloads the appropriate referral form and questionnaires (based on age and vocabulary level – please see below) and asks the parent/carer and education setting to complete them.
  3. The GP sends the referral, along with all completed questionnaires, to provide.wellbeing@nhs.net.
  4. Once received, we will confirm to the GP that the referral has been accepted and contact the family with next steps.

Questionnaires

Please note that ADHD questionnaires are not available to complete via the website – these will be shared once the patient is registered.

Autism questionnaires must be completed and submitted with the GP referral.

Each age group requires both a parent/carer questionnaire and an education questionnaire:

Please note that assessments cannot go ahead until all required forms and questionnaires have been completed and returned.
