Completed school questionaries are required as they form part of the triage process. Parents/carers are advised to continue to attempt to obtain a school questionnaire during the school holidays and to submit as soon as possible, as the process will take a little longer without this in place. It may be necessary however, for you to wait until the school reopens. If your child is not currently attending an educational setting, please contact us and we will help you navigate this.
