Understanding Ingrown Toenails and When Surgery Is Needed

What Is an Ingrown Toenail?

An ingrown toenail occurs when the edge of your toenail grows into the surrounding skin. This condition, known medically as onychocryptosis, most commonly affects the big toe. It often begins with minor irritation but can quickly become inflamed, swollen or infected.

What Causes It?

Several factors can lead to an ingrown nail:

  • Ill-fitting or narrow shoes
  • Incorrect nail trimming techniques
  • Nail trauma or repeated pressure
  • Natural nail curvature or inherited traits
  • Underlying medical conditions such as diabetes

While mild cases may improve with conservative care, persistent or painful symptoms may require surgical intervention.

Symptoms That May Indicate Surgery

You may benefit from ingrown toenail surgery if you experience:

  • Ongoing pain or tenderness at the nail edge
  • Swelling, redness or visible pus
  • Bleeding or signs of infection
  • Recurring ingrown nails, even after trimming
  • Interference with walking or footwear

The Nail Surgery Procedure: What to Expect

Step 1: Initial Assessment

Every patient begins with a consultation. Your podiatrist will:

  • Examine the nail and surrounding skin
  • Identify the severity and likely cause
  • Discuss treatment options and answer your questions

Our podiatrists are fully HCPC-registered, and each treatment is tailored to your individual case.

Step 2: Local Anaesthetic and Surgery

Once you’re comfortable, a local anaesthetic is administered to numb the area. The procedure typically includes:

  • Partial Nail Avulsion (PNA): Removing only the ingrown section of the nail.
  • Phenol ablation: Applying phenol to prevent the ingrown edge from growing back.
  • Total Nail Avulsion (TNA): Where indicated the full nail may be removed.

Most patients report minimal discomfort during or after the procedure. The entire treatment typically lasts under 30-60 minutes.

Step 3: Dressing and Aftercare

After surgery:

  • A sterile dressing is applied to protect the toe
  • You’ll receive written instructions on keeping the area clean and dry
  • Pain relief guidance is provided, although many patients feel immediate improvement
  • A follow-up appointment is usually recommended to monitor healing

Recovery Timeline

Most patients can return to work or school following post op redressing appointment. Full healing typically occurs within 2 to 4 weeks, depending on individual factors and activity level.

You’ll be advised to:

  • Wear open or loose-fitting shoes
  • Avoid high-impact exercise until the area is fully healed.
  • Watch for signs of infection or delayed healing

Additional Advice: Preventing Future Ingrown Nails

Preventing ingrown toenails often starts with simple but effective foot care habits. Nails should be trimmed straight across rather than curved at the edges, which helps prevent the nail growing into the surrounding skin. Wearing well-fitting footwear that allows enough room for your toes can also significantly reduce pressure and irritation. Good foot hygiene plays an important role too, particularly in keeping the skin healthy and reducing the risk of infection. If you notice early signs of discomfort or changes around the nail, seeing a podiatrist promptly can help address the issue before it develops into a more painful or recurring problem.
