GP Guidance: Partnering with Provide Wellbeing
Provide Wellbeing offers neurodevelopmental assessments through the NHS Essex ICB Framework and Right to Choose. Pathways are available for autism, ADHD and combined assessments for children and young people aged 6–18, as well as autism assessments for adults. We are not currently able to offer adult ADHD assessments under the NHS – for further support, please contact our team for information about the private services available.
Please follow the guidance below to help us process your patient’s referral without delays.
How to Refer
Referral methods for both NHS Right to Choose and the NHS Essex ICB Framework are outlined below. Please select the relevant pathway for your patient.

NHS Essex ICB Pathway: Referral methods
We accept referrals via the following routes:
NHS e-Referral Service (e-RS) – for children/young people assessments only
In line with local commissioning pathways, referrals can be submitted via the NHS e-Referral Service (e-RS). Please ensure both completed referral forms (available to download below) are attached at the point of submission.
Email referral – for both children/young people and adult assessments
Referrals can also be sent securely via email to provide.wellbeing@nhs.net. Please include the completed referral form(s) and any supporting documentation to enable appropriate triage – referrals submitted without the required documentation may be delayed.
For children and young people, the referral form has been split into two sections:
- Parents/carers and education professionals complete the Parent/Education Referral Form and take it to the GP
- The GP completes the GP Referral Form and submits both documents to our service via e-RS or secure email

How to find us on e-RS
To ensure your referral reaches our specific triage team, please use the following search criteria:
- Service Name: Neurodevelopment Service – Autism and ADHD – Provide Wellbeing – H6A8U
- Specialty: Children’s and Adolescent Services
- Service Location: Provide Headquarters
How to refer via e-RS
To avoid the referral being “Returned to Submitter,” please ensure the following steps are followed:
- Search for Service: Locate “Provide Wellbeing Neurodevelopmental Service” within the e-RS.
- Upload the referral documents: Use the “Add Attachment” function to upload the completed referral form along with all supporting questionnaires.
- Triage Only: Our service is listed for “Triage.” Once the completed documentation is reviewed, we will contact the family regarding their next steps.
What qualifies as a complete referral
A referral is only considered “complete” when it contains all of the following documents in one e-RS submission. Please do not submit the referral until the school and parent sections are fully completed and returned to the surgery. If we receive an incomplete referral via e-RS, we may reject the referral.
- GP/Referrer Section: Outlining clinical history and the primary reason for referral.
- Parent/Carer Questionnaire: Including birth, medical, and developmental history.
- School/Education Setting Questionnaire: Providing essential evidence of traits in the classroom. The school must complete this section.
- SNAP-IV Rating Scales: Two copies required, one from the Parent and one from the Teacher/School.
Clinical and administrative requirements
- Aged 13+ Consent: If the patient is 13 or older, they must provide their own explicit consent on the form alongside their parent/carer.
- Parental Responsibility: Please list everyone with parental responsibility, if parents live apart and confirm if we have consent to share information with both parties.
- Existing Private Reports: If a family has a previous private diagnosis, the ICB requires that the full report be attached to the e-RS or email referral for clinical review.
GP letter template: managing parent expectations
One of the most common reasons for referral delay is a missing School Questionnaire. You can use the text below to help parents coordinate with their school:
“To help your GP submit a successful referral to Provide Wellbeing, we need a complete ‘referral documentation.’ This must include the School Questionnaire and the Teacher’s SNAP-IV scale. Please take this pack to your school’s SENCO or a teacher. Once they have completed their section, you have to return the entire form to the surgery so we can upload it to the NHS e-Referral Service (e-RS).”
