GP Guidance: Referring to Provide
Wellbeing (MSE Pathway)

Provide Wellbeing is a commissioned provider for neurodevelopmental assessments across Mid and South Essex for children and young people aged 6 – 18.

To ensure your patient’s referral is triaged effectively, please follow the mandatory MSE ICB Framework and submission protocols below.

Mandatory Referral Method: e-RS

Important Notice: In line with local commissioning requirements, Provide Wellbeing only accepts referrals via the NHS e-Referral Service (e-RS).

  • Referrals sent via email, post, or My Care Bridge will not be processed.
  • GPs must submit the referral through e-RS and ensure all required documentation is attached at the point of submission.

How to Refer via e-RS

To avoid the referral being “Returned to Submitter,” please ensure the following steps are followed:

  1. Search for Service: Locate “Provide Wellbeing Neurodevelopmental Service” within the e-RS.
  2. Upload the Clinical Packet: Use the “Add Attachment” function to upload the completed Standard MSE CYP Referral Form along with all supporting questionnaires.
  3. Triage Only: Our service is listed for “Triage.” Once the completed documentation is reviewed, we will contact the family regarding their next steps.

The “Right First Time” Checklist

A referral is only considered “complete” when it contains all of the following documents in one e-RS submission. Please do not submit the referral until the school and parent sections are fully completed and returned to the surgery. If we receive an incomplete referral via e-RS, we may reject the referral.

  • GP/Referrer Section: Outlining clinical history and the primary reason for referral.
  • Parent/Carer Questionnaire: Including birth, medical, and developmental history.
  • School/Education Setting Questionnaire: Providing essential evidence of traits in the classroom. The school must complete this section. 
  • SNAP-IV Rating Scales: Two copies required, one from the Parent and one from the Teacher/School.

Clinical & Administrative Requirements

  • Aged 13+ Consent: If the patient is 13 or older, they must provide their own explicit consent on the form alongside their parent/carer.
  • Parental Responsibility: Please list everyone with parental responsibility. If parents live apart and confirm if we have consent to share information with both parties.
  • Existing Private Reports: If a family has a previous private diagnosis, the ICB requires that the full report be attached to the e-RS referral for clinical review.

GP Letter Template: Managing Parent Expectations

One of the most common reasons for referral delay is a missing School Questionnaire. You can use the text below to help parents coordinate with their school:

“To help your GP submit a successful referral to Provide Wellbeing, we need a complete ‘evidence pack.’ This must include the School Questionnaire and the Teacher’s SNAP-IV scale. Please take this pack to your school’s SENCO or a teacher. Once they have completed their section, you have to return the entire form to the surgery so we can upload it to the NHS e-Referral Service (e-RS).”
